Discharge Planning: Closing the Revolving Doors Between Homelessness and Institutions

Homeless individuals often cycle among hospitals and correctional institutions, shelters and the streets. These “revolving doors” have been known to exacerbate homelessness.
Regardless of which institution an individual may be leaving, discharge planning is imperative to assure a successful transition. Without a stable home environment and family or peer support, individuals recovering from illness and those newly emancipated from correctional institutions are especially vulnerable to the harsh realities of homelessness.
Panelists will discuss how successful discharge planning can ensure a successful transition from institutions to the community and serve as the lynchpin of a comprehensive homelessness prevention strategy.
